Job Summary:
Reporting to the Area Manager the Licensed Practical Nurse position will be responsible for performing assigned duties as a member of the Home Care team. The person in this position will work with individuals and their families who cannot function independently in the community; by mobilizing and managing health and social resources required.

Qualifications:
- Previous recent experience (obtained within the last five years) is desired. In absence of recent experience, completion of a refresher program or recent graduation will be considered.
- Current LPN registration.
- Heart saver competency. Medication administration module required.
- Completion of the medication administration module required.
- Previous community related experience is a definite asset.
- Excellent interpersonal/communication skills and organizational skills.
- A valid driver�s license and access to a reliable vehicle.
